Imagine if game design programming for most open-world or MMO-type games became so uniformly coded that they were fully inter-compatible, and the servers running these games became so huge that all these different games types were running together off single servers.
At that point, it wouldn't be very difficult for you to run your Skyrim character off the edge of the map and into an Assassins Creed world, or a Witcher setting.
There's already some small steps towards this with game modding, bringing items, spells, and visual sprites into the game world.
It would be similar to that "Ready Player One" movie.
